What Is an Employee Evaluation Template?

An employee evaluation template is a simple, structured form that managers can use to review performance over a set period.

Instead of starting from scratch, this template gives you a consistent way to record strengths, track areas for improvement, and set goals for the next review cycle. It also helps employees clearly see how their work is being assessed.

Why Is Employee Evaluation Important?

Regular evaluations keep the review process fair and consistent. When everyone is measured against the same criteria, feedback feels more transparent and objective.

These evaluations also create a written record that can guide decisions on promotions, salary adjustments, or training opportunities. Just as importantly, they open up conversations between managers and employees so both sides can align on expectations and development plans.

What’s Included in an Employee Evaluation Template?

A standard evaluation template usually covers:

  • Basic employee details such as name, department, job title, and review date
  • A clear rating scale for scoring performance
  • Categories like communication, teamwork, continuous improvement, time management, and strategic thinking
  • Specific criteria within each category, with space for comments and ratings
  • A final grade and bonus eligibility (if applicable)
  • Signatures from both manager and employee to confirm acknowledgment

This format makes the process easier to follow and ensures that all key points are documented in one place.

Second Tab for Employee Self-Assessment

The template also includes a Wellbeing and Self-Reflection Chart that employees can complete on their own. This section encourages them to share how they’ve been feeling during the review period and to reflect on their personal progress.

Employees can use it to:

  • Note their energy, motivation, and overall mood
  • Record habits or practices they focused on
  • Highlight achievements or wins they are proud of
  • Identify common challenges or distractions
  • Set intentions for what they want to improve next

By combining feedback from managers with employee self-reflection, the evaluation becomes more balanced and meaningful. It’s not only about measuring past performance, but also about planning ahead and agreeing on what to work toward in the future.
